    The Challenge

Paid ads bring predictable lead flow, but every lead has a price tag attached. For surgical practices selling $20,000-$50,000 treatment plans, the highest-intent patients often start with a Google search, not an ad click. Both a premium dental implant provider ("Practice A") and Galleria Oral & Maxillofacial Surgery needed websites that could win the search results that matter, built from thin content and weak technical foundations into real organic assets.

On top of classic SEO, a new front had opened: AI search. Patients now ask ChatGPT, Gemini, and Google's AI Overviews who they should trust with their surgery. If your practice isn't cited in those answers, you don't exist in that conversation.

The goal: build organic visibility from the ground up for two practices, with zero paid search spend, and make both sites citable sources for AI search engines.

    What I Built

    Technical Foundation First

Both websites were built and structured for search from day one:

    • Site architecture organized around procedure and condition pages that mirror how patients actually search
    • Schema markup (medical organization, physician, FAQ, and procedure structured data) so search engines and AI models can parse who does what, where
    • Core Web Vitals optimization, fast, mobile-first pages that hold rankings once earned

    Content Built Around Patient Intent

    • Procedure pages written to answer the questions patients ask before a $30,000 decision: cost, recovery, risks, alternatives
    • Location-focused pages to capture "near me" and city-level searches
    • FAQ content structured to win featured snippets and AI Overview citations

    Authority Building

    • 1.5K backlinks from 380 referring domains built for Practice A through outreach, citations, and content worth linking to
    • 322 backlinks from 142 referring domains for Galleria OMS
    • Consistent NAP citations and local profile optimization to reinforce the sites as the canonical source for each practice

    Optimizing for AI Search

This is where the compounding payoff showed up. By treating AI engines as a distinct channel, clean structured data, quotable answer-format content, consistent entity signals, both practices became cited sources:

    • Practice A: 27 mentions and 102 cited pages across ChatGPT, Gemini, AI Overviews, and AI Mode, with its own domain as the #1 cited source in its niche
    • Galleria OMS: AI visibility score of 28, mentioned across all four major AI search surfaces

    The Results

    Practice A, 6 Months

    Practice A Semrush organic growth

    • Organic traffic up +51% to ~1,800 monthly visits, a steady climb with no plateau
    • Organic keywords up +40% to ~1,900, with a growing share in top-3 positions
    • 45% traffic share in its competitive set
    • 93.6% of visibility from pure organic rankings, not dependent on any single SERP feature

    Galleria OMS, 12 Months

    Galleria OMS Semrush organic growth

    • Organic keyword footprint grew +8.4% year over year to 387 keywords, with top-3 rankings climbing steadily quarter over quarter
    • A stable base of ~1,100 monthly organic visits and 41% traffic share in its market
    • Higher AI visibility (28) than Practice A, proof the AI-search playbook transfers across practices

    Why It Matters

Every organic visit to these sites is a patient the practice didn't pay an ad platform to reach. At implant-level case values, even a handful of organic consultations per month pays for the entire SEO program many times over.

More importantly, organic and AI visibility compound. Paid traffic stops the moment you pause spend. These rankings, backlinks, and AI citations keep working, the traffic curves in the Semrush data don't dip when ad budgets shift.

    The Takeaway

SEO for high-ticket healthcare isn't about blog volume, it's about becoming the source that both Google and AI engines trust for a narrow, high-value set of questions. Technical foundations, intent-matched content, real authority signals, and early positioning in AI search turned two new websites into durable patient acquisition channels with zero ongoing media cost.
